Profile Personal Information

Overview

Personal Information stores important personal information about the person you are helping such as name, email address and phone number so that you can easily view that information.

Additional fields, such as ID number, address, or zip code can be added for Professional and Enterprise customers to track even more information.

Information is stored securely and access to that information is controlled by the Customer to protect the Seeker's Personally Identifiable Information (PII).

How it Works

Seeker Profiles are automatically created any time a referral is made/received, or an Assessment has been completed for someone you're helping (Seeker). At that point, the Seeker's Profile is available to you in their People I’m Helping Dashboard, and it will contain the Personal Information that was used to complete the referral or Assessment.

Personal Information appears in the left sidebar on the Seeker Profile, and shows all the most recent contact info that you or your team has entered about the Seeker. This data is editable by anyone who has access to the Seeker Profile. Any referrals or Assessments submitted with new information will update the Seeker’s Personal Information.

Personal Information can be edited in the Seeker Profile Information pane by clicking Edit Personal Info.

The People I’m Helping Dashboard refers to these fields to allow you to search by name, phone number, email address, or ID number to look up Seekers you have previously helped. For Customers with Custom Seeker Profile Fields, the People I’m Helping Dashboard can also be filtered by distinct field types and date fields.

When making new referrals of Assessments, Personal Information can be pulled into respective fields for efficiency.

Standard Fields

The standard fields in the Personal Information that are available to all users are:

  • First Name

  • Last Name

  • Email

  • Phone Number

Custom Seeker Profile Fields & Unique Identifiers

Professional and Enterprise Customers can create additional fields called Custom Seeker Profile Fields. These fields can only be viewed and updated by Workers and Site Admins on a Customer Site. Some examples are:

  • Unique ID numbers like Patient ID, Medicaid ID, Medical Record Number

  • Preferred Language

  • Date of Birth

  • Navigation or Follow up Dates

  • Stage of Navigation (new, in navigation, closed)

Custom Seeker Profile Fields are limited to a single Customer Site and cannot be displayed across Customer Sites. Only standard fields will show across sites.

 Custom Seeker Profile Fields can be anything except Social Security Number.

 

Unique Identifier

A unique identifier is a way of connecting Assessments, Referrals and Appointments to a single Seeker, even if they have different contact information. 

Identifiers must uniquely identify a specific user

EXAMPLE:

  •  An ID number may be an identifier, because there is only one ID associated with that person.

  •  By contrast, date of birth is not an identifier because multiple people can have the same birthday.

Unique Identifiers are searchable in the People I’m Helping Dashboard and will appear on as optional fields in the Connect form, for logged in Site Workers.

Profile Field Formats and Functionality

Profile Fields can have different formats and functionality:

  1. Dates

    • Can be filtered on the People I’m Helping Dashboard.

  2. Multiple Choice

    • Fields that are multiple choice can be added as a dropdown filter on the People I’m Helping Dashboard.

      • EXAMPLE: Stage of Navigation (new, in navigation, closed)

  3. Text Fields

    • Unique Identifiers can be searched for on the People I’m Helping Dashboard.

      • EXAMPLE: Patient ID
    • Other free text Custom Seeker Profile Fields that are not identifiers cannot currently be searched for.

      • EXAMPLE: Street address

Autofilling

When in Seeker Context, information from the Personal Information section will autofill:

  • Connect/Referral Forms
  • Assessments
